Environmental Services
The Environmental Services Department performs a wide range of
services, ranging from cleaning and maintenance to security and
mail, as well as providing support for all community relations
programs.

In addition to cleaning services for the patients, visitors, and
staff of Holzer Medical Center, Jackson, the Environmental Services
Department is proud to participate and help support many hospital

most versatile departments at Holzer Medical Center, Jackson.

Nutrition Services
The Nutrition Department at Holzer Medical Center, Jackson works
to ensure the nutritional needs and concerns of our patients are
met. We identify and develop plans for correcting malnutrition, as
well as providing dietary education for the Medical Nutrition
Therapy of certain diagnoses.

Nutrition Services
At the Nutrition Department, our duties include coordinating food
service operations for inpatients and outpatients, as well as
serving all hospital cafeteria and catering functions. We also
provide inpatient and outpatient dietetic counseling for Medical
Nutrition Therapy.
- Services to inpatients are based on Registered Nurse (RN)
and/or MD consults.
- Referrals for outpatients are with a Physician Rx only.
- All inpatients are screened and typically assessed by RD/DTR
within an appropriate period after admission.
Nutrition Services manages nutritional therapy for wide
population of patients, ranging from pediatrics to geriatrics, and
from critically ill, ventilator-dependent patients to those
receiving treatment for COPD, CHF, Diabetes, and more.
We provide monthly
Diabetes Education Classes in collaboration with Holzer Medical
Center, Gallipolis (Nancy Stevens RN,CDE), as well as Nutrition
Support (TPN/PPN, TF, etc.).
Department Personnel
The Nutrition Services Department is staffed with Registered
Dietitian (RD), a Dietetic Technician, registered (DTR), both
credentialed by the Commission on Dietetic Registration (CDR) and
with appropriate licensure for RD.

 The Nutrition Department at Holzer Medical Center, Jackson serves
the residents of Jackson, Oak Hill, Albany, Glade, McArthur,
Wellston, Wilkesville, and other nearby communities.
Insurance/Payment
We are able to accept Medicare and Medicaid, various types of
personal insurance, and self-pay.

Health Information Management
At Health Information Management, we process medical records for Holzer Medical Center Jackson. We handle all releases of Information for the hospital.

The hours of operation are Monday – Friday 8:00 a.m. – 4:30 p.m.

Services
We provide copies of medical records for personal review, continuity of care, and other needs. Our Medical Records are scanned into a computerized system and can be easily accessed when requested. This state-of-the-art system allows us to process your request in a timely manner.
To request a copy of your medical records, you can call Health Information Management, or stop in at the Holzer Medical Center, Jackson to fill out a release of information. Certain fees may apply. All requests for hospital medical records are processed from this location.
The HIM Department does not handle medical records for the Holzer Clinic.
While the HIM Department does have offices in the building at 2881 State Route 160, there is limited access at that location, so please stop by one of our hospital offices.
Holzer Medical Center, Jackson: The HIM Department is located on the first floor of HMC-J. This location handles Release of Information and the Incomplete Area for Physician’s.

The Health Information Management Department provides services to Holzer Medical Center, Gallipolis and Holzer Medical Center, Jackson. Our service area includes Gallia County, Jackson County, Meigs County, Athens County, Lawrence County, Ross County and Pike County, Scioto County, Vinton County, and Washington County in Ohio, Mason County, Braxton County, Wood County, Cabell County in WV, Greenup, KY. Cities include Gallipolis, Jackson, Wellston, Pomeroy, Columbus, Chillicothe, Waverly, Piketon, Marietta Ohio, Point Pleasant, Huntington, Sutton, Parkersburg in WV, and Ashland KY.
