Staff member - Social Services JacksonSocial Services assists patients in obtaining home health services, long or short-term residential placement, inpatient rehabilitation, durable medical equipment, prescription medication discounts, advanced care directives, in-home assistance via the Area Agency on Aging, CareStar, and more. We help patients and their families cope with the many challenges that accompany illness, disability, and other life changes.

At the Social Services department of Holzer Medical Center - Jackson, our staff enables patients and their families to receive entitled services which assist in rehabilitation, as well as helping them live in the least restrictive environment.

Social Services at HMC Jackson

The Social Services department is committed to supporting our patients and their families meet and manage the challenges they face. Social workers provide a range of services, specific to the needs and situations of each patient and their families, including:

  • Linking patients and their families with appropriate community resources
  • Providing information on outpatient therapies and surgeries
  • Assistance with patients visiting physicians located at HMC, Jackson
  • Help understanding and utilizing services such as the Emergency Department, Express Care, and Outpatient Services.

At Social Services, Jackson, we largely serve elderly and disabled patients either living at home or who desire to return home with assistance from community agencies, or after a short-term stay at a local Skilled Nursing Facility or inpatient rehabilitation center. We also serve patients from the Holzer Medical Center, Jackson Emergency Room or HMC Physicians who are in need of information on community services.

Frequently Asked Questions

Will you see to it that my family member is placed in a nursing facility?

When a patient is unwilling to enter a nursing facility, and proves to be alert and oriented and has not been declared incompetent, it is not possible for us to place them into a nursing facility against their will.

May I complete advanced directives for a family member?

No. Advanced care directives may only be completed for oneself.

Can you help with wills and financial power of attorney?

No. Social Services is unable to assist patients with wills or financial powers of attorney.

Will Medicare pay for a stay in a Skilled Nursing Facility?

Many patients are unaware that Medicare will pay for a 20 day stay in a Skilled Nursing Facility with a qualifying hospital stay.

Theda Covey LSW has a Bachelors Degree in Social Work and is licensed by the State of Ohio. She has 34 years of experience.
