Patients Served by the Social Services DepartmentAt Social Services, Jackson, we largely serve elderly and disabled patients either living at home or who desire to return home with assistance from community agencies, or after a short-term stay at a local Skilled Nursing Facility or inpatient rehabilitation center. We also serve patients from the Holzer Medical Center, Jackson Emergency Room or HMC Physicians who are in need of information on community services. Frequently Asked QuestionsWill you see to it that my family member is placed in a nursing facility? When a patient is unwilling to enter a nursing facility, and proves to be alert and oriented and has not been declared incompetent, it is not possible for us to place them into a nursing facility against their will. May I complete advanced directives for a family member? No. Advanced care directives may only be completed for oneself. No. Social Services is unable to assist patients with wills or financial powers of attorney. Will Medicare pay for a stay in a Skilled Nursing Facility? Many patients are unaware that Medicare will pay for a 20 day stay in a Skilled Nursing Facility with a qualifying hospital stay.
