How do I contact hospice?

For initial referrals you may call our office Monday through Friday from 8:00 am to 4:30 pm at 740-446-5074, or toll free in OH 1-800-500-4850. You may also call the Holzer Medical Center Operator after hours at 740-446-5000 and the operator will page the nurse on call.

When a patient is admitted to the hospice program, we place a folder in each patient’s home with pertinent information in it for the patient and/or their family. The above numbers and details about placing a call to hospice staff are listed in the front of this folder.

How is Hospice care covered?

We accept Medicare, Medicaid, and most private insurances. Holzer Hospice does not decline to admit any patient who meets hospice criteria regardless of ability to pay.

What diagnoses are covered under hospice care?

Diagnoses covered under hospice care include but are not necessarily limited to cancer, stroke, ALS (amyotrophic lateral sclerosis), multiple sclerosis, renal failure, liver disease, pulmonary disease, heart disease, dementia/Alzheimers, Adult Failure to Thrive, and Parkinson’s disease. Patient’s are also eligible if they meet some of these diagnosis criteria with significant co-morbidities, or rapid decline suggesting a six-month or less prognosis.

Who can make a referral to hospice?

Anyone. Your doctor. A family member. You yourself. The hospice nurse will collect the data to determine eligibility and help you through the process of admission when applicable.
