Retirement
401(a) Retirement Plan provides discretionary employer contributions
- Eligible upon completion of one year of service and 1,000 hours worked
- Fully vested after 5 years of employment
- Employees can self-direct investments or choose from a menu of funds
403(b) Employee Savings Plan allows employees to make contributions
- Eligible immediately upon hire
- Contributions can be made up to the IRS allowable maximum
- Option to elect contributions pre-tax, post-tax (ROTH), or a combination of the two
- Employees are always 100% vested in their contributions

457(b) Deferred Compensation Savings Plan allows physicians to save more for retirement through pre-tax payroll deductions

Health Benefits
Health Plan
- Holzer offers a PPO health plan through MedBen; Holzer pays approximately 80% of the premium. Employee portion ranges from $148 per month for employee-only coverage to $564 per month for family coverage.
- Employees who have Holzer's MedBen Plan (or a comparable insurance plan) are eligible for discounts on outpatient services - 100% after insurance for employees, spouse and dependent children.
Dental Options
- Option 1 provides preventive and basic services at a lower monthly premium. Best for individuals/families who need little to no dental care outside of their oral exams and cleaning.
- Option 2 covers major services in addition to preventive and basic for a higher premium.
Vision
- Provided by VSP with access to a nationwide network.
Life, Disability, and Long-Term Care Plans
Employer Paid Basic Life and Accidental Death and Dismemberment (AD&D)
- Holzer provides coverage in the amount of $300,000
Employer Paid Short-Term Disability (STD)
- Beginning on the 11th calendar day off work, 100% of pay for 30 calendar days. Physician must use available ATO days for each day missed, with the exception of maternity leave
Employer Paid Long-Term Disability (LTD)
- Holzer pays a premium for a benefit of 60% of salary to a maximum benefit of $7,000 per month starting the 91st calendar day off work
Long-Term Care Insurance
- Physician may elect to purchase long-term care insurance for self and/or spouse, parents, grandparents, siblings, in-laws, and adult children

Support for Board Certification/Re-Certification
- Reimbursed up to $1,500 (once per testing cycle) for board certification or re-certification testing fees
- Receive up to 8 days off (once per testing cycle) for board study
Licensure and DEA Reimbursement
- Holzer reimburses physicians for medical licensure and DEA renewals (initial cost is the responsibility of the employee)
